Description:

Stream 1: This Stream provides funding to PHNs for Corporate Governance, Health Systems Improvement and Flexible Funding.

Stream 2: This Stream funds PHNs to develop resources to assist unaccredited practices in their regions to achieve and maintain accreditation under the NGPA Scheme.

Stream 3: This Stream funds PHNs to work with Residential Aged Care Homes (RACHs), GPs, practices and/or Aboriginal Community Controlled Health Services to increase the access of RACH residents to primary care services.

Stream 4: This Stream funds PHNs to support general practices to implement effective models of multidisciplinary team care.

Stream 5: This Stream funding allows PHNs to continue to employ the additional staff required to carry out a range of administrative activities to support the National COVID-19 vaccination program.

Stream 6: This Stream aims to strengthen the role PHNs play in commissioning multidisciplinary health care to improve the management of chronic conditions and reduce avoidable hospitalisations.

Stream 7: This Stream funds selected PHNs in regions where market failure is an impediment to the matching of GPs and practices with residents in RACHs. Only 18 selected PHNs eligible to apply.

Eligibility:

Grants under the Primary Health Network (PHN) Program are limited to existing PHNs only. This is in keeping with the intent of the program to provide a national network of regional organisations that commission and integrate services at the local level to address identified needs and provide support and system level integration for health services to improve coordination of care for patients.

PHNs are individual, not-for-profit organisations selected through an open competitive process in 2014-15 to deliver the PHN Program. PHNs must meet the requirements of the grant agreement and are required to demonstrate their performance against the PHN Program Performance and Quality Framework, which is assessed annually. The yearly performance assessment determines the continuation of grant funding to each PHN. Grant applicants must be one of the 31 PHNs listed in Section 4 of the Grant Opportunity Guidelines to be eligible for these grant opportunities.

PHNs undertake Needs Assessments and use collaborative approaches to determine the priorities for the program. They have experience in commissioning services based on this assessment that reflect their local circumstances which enhance the capacity, efficiency, and effectiveness of after-hours services. 

Grants under this grant opportunity will only be offered to eligible PHNs to deliver on the program objectives and commission health services within the boundaries of each PHN.
