Description:

The purpose of the Registered Nurse Led Mobile Health Care Clinics is to provide funding to the Health on the Streets project

led by primary and mental health trained nurses to help individuals navigate access to services.

 

The objectives of the program are:

• To deliver a mobile primary care service targeted at homeless people; and

• Improve access to the range and choice of primary care and mental health supports available to target population groups including disadvantaged groups including, disadvantaged groups Indigenous populations, and people in regional rural and remote locations.

 

The intended outcomes of the program are:

• Improved coordination of care for patients at risk of poor health outcomes are met through efficient and effective health care services.

• Support provision of primary care and mental health services to people experiencing homelessness, where there is no general practitioner or other specialist support available; and

• Improved health of vulnerable people in the central coast through providing accessible primary care services.
