Advance Care Planning
This grant opportunity is part of the National Palliative Care Projects, which align with the Primary Health Care Quality and Coordination Program’s objectives and outcomes through the National Palliative Care Strategy 2018.
This grant opportunity focuses on two action items from the Implementation Plan for the National Palliative Care Strategy:
• Action Area Two: The collaboration and coordination of palliative care is improved.
• Action Area Three: Advance care plans are being prepared by people affected by life-limiting illnesses and used to facilitate shared decision-making across care settings. (Note: Advance care planning is not limited to those with a life-limiting illness).
The intended outcomes of the Advance Care Planning Grant Opportunity are:
• Measurable improved uptake and use of advance care planning documents nationally, including among underserved population groups.
• Increased understanding and awareness of advance care planning in the community and among the health and aged care workforce.
• Individuals across Australia feel supported to document and communicate their end-of-life care choices with their families, carers and health professionals.
• Effective coordination of advance care planning documents between care settings and professionals.
• Increased capability of the health and aged care workforce to support advance care planning as a result of national training.
• Improved understanding of the national prevalence of advance care planning and future steps to address challenges.
• Improved data and measurement of National advance care planning activities.

Eligible activities must directly reflect the outcomes and objectives of this grant opportunity and may include:
• Implementing strategies to promote and increase the uptake and use of advance care planning documents.
• Service provider skills development for advance care planning through:
- Education and training for frontline health, aged care and disability providers
- Identifying and implementing service models that improve collaboration and coordination of advance care planning
- Strategies to improve sharing of advance care planning documents during transitions of care between health, aged care and disability sectors.
• Activities that strengthen the understanding and awareness of advance care planning among the community and health and aged care workforce:
- Provision of advance care planning advice and support.
- Promoting the benefits and need for advance care planning.
• National advance care planning prevalence research/studies.
The activities must also be national and be accessible in all states and territories in Australia.
