The Indigenous Australians’ Health Programme (IAHP) aims to increase access to culturally appropriate and self-determined health care by prioritising Aboriginal Community Controlled Health Services (ACCHSs) to lead the delivery of programs and services. 

This grant opportunity’s purpose is to provide the National Aboriginal Community Controlled Health Organisation (NACCHO) with funding to deliver the Community Controlled Transition Care (CCTC) program. 

The CCTC program will involve the co-design and delivery of a community controlled transition care model for First Nations peoples exiting custody. It will seek to improve health care access and outcomes for First Nations people in the custodial system, providing First Nations peoples with improved discharge planning, reintegration support and culturally safe health care post-release.

The Australian Government has announced a total of $14,497,810 over 3 years for the CCTC grant opportunity. 

The grant opportunity will run from November 2026 to June 2029.

The eligible organisation was determined based on its demonstrated capability to deliver the project, supported by existing infrastructure, governance, and stakeholder relationships. As the national peak body for ACCHS sector, NACCHO is uniquely positioned to develop and coordinate the delivery of an ACCHS-led transition care model. NACCHO has been assessed by the department to have:

• demonstrated experience, knowledge of and capability to deliver the grant objectives and outcomes – NACCHO is the national peak body for the ACCHS sector and since at least 2023 has worked closely with the department on custodial health care reform. NACCHO played a key role in the developing the National Review of First Nations Health Care in Prisons and its Implementation Approach

• existing infrastructure and relationships to support the specified grant activities – including existing relationships with 148 ACCHSs that NACCHO formally represents, along with Commonwealth and State and Territory Governments

• an established and proven track record – NACCHO has an established, longstanding leadership role in delivering culturally safe comprehensive primary health care services to First Nations peoples across Australia.

A closed non-competitive selection process was chosen to ensure continuity, minimise duplication, and maximise the impact of public investment by leveraging the expertise, infrastructure, and performance history of the most suitable provider.
