Description:

The Australian Government is committed to delivering support for people living with rare and complex diseases to better navigate the health system, including through the assistance of telehealth nurses.

The Australian Government has announced $3 million to provide support to navigate the health system for people living with rare and complex diseases, including through the assistance of telehealth nurses. The activity will run over three years from late 2022-23, and this grant opportunity comprises funding of up to $2.48 million over those three years.

Alongside this grant, an independent evaluation will be conducted. The independent evaluation will run concurrent with the program and aims to establish a fit-for-purpose monitoring and evaluation framework to ensure outcomes are measurable and meaningful and supported by appropriate program performance metrics. The evaluation will provide evidence to the Australian Government on the individual and system impact, cost-effectiveness and sustainability of the patient navigation model trialled under the program.

An independent advisory group will also provide oversight of the development and implementation of the program. Its membership will be proposed by the lead organisation and agreed by the department.

This grant opportunity forms part of the department of Health and Aged Care’s Outcome 1, Program 1.6 – Primary Health Care Quality and Coordination, which aims to strengthen primary health care by delivering funding to frontline primary healthcare services and improving the access, delivery, quality and coordination of those services. This will help improve health outcomes for patients, particularly people with chronic and/or mental health conditions, and assist in reducing unnecessary hospital visits and admissions.

Eligibility:

To be eligible you must be one of the following entity types:

  • Indigenous Corporation
  • Company
  • Cooperative
  • Incorporated Association
  • Partnership

If you are applying as a Trustee on behalf of a Trust, the Trustee must have an eligible entity type as listed above.

Applications from consortia are acceptable, as long as you have a lead applicant who is solely accountable to the Commonwealth for the delivery of grant activities and is an eligible entity as per the list above. Eligible organisations can form a consortium with organisations outside of the types listed above.

If you are successful, relevant personnel working on the grant activity must maintain the following:

  • Working with Children check; and
  • Working with Vulnerable People registration (or equivalent depending on the relevant state or territory).

The department recognises the Government’s response to the Royal Commission into Institutional Responses to Child Sexual Abuse, and the Commonwealth Child Safe Framework.

As such, the department expects that all grant recipients comply with all Australian law relating to employing or engaging people who work or volunteer with children. This includes working with children checks and mandatory reporting; and the department may request an annual statement of compliance with this requirement. If successful, these details will be outlined in the successful organisation’s grant agreement.
