Eligibility:

This grant opportunity has been established as a one-off or ad hoc grant. The department considers that this is an appropriate type of selection process considering there is urgent need for the PHNs to be able to continue the provision of services for improving access to primary care by vulnerable populations. Grants under the PHN Multicultural Access Program are limited to existing PHNs only. You must be one of the 31 PHNs listed in the Grant Opportunity Guidelines to be eligible.
