The National Psychosocial Support –Transition (NPS-T) program aims to support the transition of former Commonwealth community mental health clients to the NDIS or where deemed ineligible, to the Continuity of Support program from 1 July 2019 to 30 June 2021.

The objectives of the NPS-T program are to:

• Provide non-clinical support for people with severe mental illness and associated psychosocial functional impairment;
• Support the transition of clients to test their eligibility for the NDIS to ongoing support arrangements under the NDIS or to the CoS program;
• Reduce the avoidable need for more intensive and acute mental health services and enhance the appropriate and optimal use of the health care system.

Only the listed organisations at Section 4 of these guidelines are eligible for funding
