The purpose of the General Practitioner Led Mobile Health Care Clinics is to provide funding to the Street Side Medics organisation to support increasing access to mobile medical services to those who are vulnerable and experiencing homelessness. 

The objectives of the program are:

• Deliver mobile medical clinics providing primary care services targeted at homeless people;

• Reduce cost barriers for people experiencing homelessness by providing services of no personal charge to patients;

• Improve patient participation by partnering with relevant local services in areas with demographically high concentrations of homelessness; and

• Improve access to the range and choice of medical and health care services available to target population groups including children, disadvantaged groups, Indigenous populations, and people in regional, rural and remote locations.

    The intended outcomes of the grant are:

• Improved coordination of care for patients at risk of poor health outcomes are met through efficient and effective health care services in the after-hours period;

• Support provision of primary care services to people experiencing homelessness, where there is no general practice or other specialist support available; and

• Improved health in vulnerable people through providing accessible primary care services.
