Description:

The purpose of this grant opportunity is to ensure service continuity of psychosocial support services to assist people with severe mental illness, who have an associated reduction in psychosocial functional capacity and are not receiving similar supports through state or territory programs, or through the NDIS. The CPS Program has been in place since 1 July 2021, and this grant opportunity will run over two years from 1 July 2023 to 30 June 2025. 

 

CPS Program activities will continue to be delivered by PHNs. PHNs will provide: 

• a regional approach to targeting psychosocial support services to individual needs, promoting flexible, efficient service delivery. Services may include individual and group support, and peer support; and 

• improved service coordination for individuals with severe mental illness and associated psychosocial functional impairment, while taking into account supports available across all levels of governments, the community and relevant sectors.

Eligibility:

Grants under the Primary Health Network (PHN) Program are limited to existing PHNs only. This is in keeping with the intent of the program to provide a national network of regional organisations that commission and integrate services at the local level to address identified needs and provide support and system level integration for health services to improve coordination of care for patients. 

 

PHNs are individual, not-for-profit organisations selected through an open competitive process in 2014-15 to deliver the PHN Program. The yearly performance assessment determines the continuation of grant funding to each PHN. Grant applicants must be one of the 31 PHNs listed under item 4.1 of the Grant Opportunity Guidelines to be eligible for grants under the PHN Program.
