Primary Health Care Development Program - Bribie Island Pilot
The purpose of the grant is to improve access to after hour services for the residents of Bribie Island. This grant opportunity is to support Brisbane North Primary Health Network (PHN), which falls within the Bribie Island catchment area to support the co-design, implementation and evaluation of the pilot.
The objectives of the grant are to design, commission, implement and evaluate a model for service delivery of primary care after hours services over a four year period to 30 June 2024.
To achieve these objectives Brisbane North Primary Health Network (Brisbane North PHN) will:
· undertake a stakeholder consultation process to ensure needs of the population are considered when developing a model for service delivery to improve access and quality to after hours primary care services on Bribie Island;
· co-design and commission an optimal model for service delivery using blended funding model consisting of block payment and MBS fee-for-service;
·improve access to primary care after hours services on Bribie Island, through the implementation of a service delivery model; and
·conduct an evaluation to determine the effectiveness of the implemented model to justify the need for future funding for after hours services on Bribie Island.
As this is a closed non-competitive grant opportunity only Brisbane North Primary Health Network are eligible to apply.
