Activating primary care COPD patients with Multi-morbidity (APCOM) study
Purpose:
Chronic Obstructive Pulmonary Disease (COPD) is a common chronic condition that can progress to disability and death. Self-management support for people with COPD can reduce exacerbations and improve quality of life but many programs are not effective as they fail to recognise the impact of other long term conditions. This study will evaluate the effectiveness, cost-effectiveness and uptake of a self-management intervention that is tailored to individual need and recognises multimorbidity.
