Increasing days living in the community and improving quality of life among people living with dementia and their carers
Purpose:
There is a need to support people living with dementia to connect with needed health care and other services and proactively plan their care. This study will test the impact of a nurse led intervention on increasing the days spent in the community, and the quality of life of people living with dementia and their family carers using a randomised controlled design. Outcomes will be assessed for both the person with dementia and their family carer at baseline, 6 and 12 month follow up.
