Total Cardiac Care - STROKE: A randomised controlled trial of a comprehensive smartphone application-centric model of care to improve outcomes in stroke patients.
Purpose:
One in three patients are re-admitted to hospital within 6 months of successful treatment for stroke. We have developed a smartphone-based ‘hospital in the home’ system that allows patients, GPs and specialists to monitor blood pressure, heart rhythm, activity, and medication use daily. Patients and medical teams are alerted when signs indicate treatment changes are needed. Our study is aimed at proving this system can reduce the number of patients re-admitted to hospital after stroke.
