Commonwealth Home Support Programme – Better Ageing – Promoting Independent Living
The Commonwealth Home Support Programme aims to support frail, older people aged 65 years and over (or aged 50 years and over for Aboriginal and Torres Strait Islander people) to live as independently as possible in their own homes and community for as long as they can and wish to do so.
A trial is currently underway through five RAS organisations across 20 Aged Care Planning Regions (ACPR) in New South Wales, Queensland, South Australia, Tasmania and the Northern Territory. The intended objective of this grant opportunity is to offer additional funding to existing CHSP providers operating in the Trial site regions that deliver Allied Health and Therapy Services.
The purpose is to help CHSP providers meet the increased costs of delivering a greater number of Allied Health and Therapy Services to consumers in the Trial site regions.
The intended outcomes of this grant opportunity are:
- Older people meet their reablement goals through better access to Allied Health and Therapy Services; and
- Ensuring older people stay more independent at home and active in the community thereby enhancing their quality of life and/or preventing or delaying their admission to long-term residential care.
As this is a closed non-competitive grant opportunity only organisations listed in Attachment C1 of the grant opportunity guidelines are eligible.
